Jake was seen recently for an ear infection by Dr. Blake, his pediatrician. While Dr. Blake was checking out Jake's ears, he inquired how Jake was doing in general. He remembered from Jake's one-year check up that Jake was just about to start walking. As Karen, Jake's mom, started talking to Dr. Blake, she realized just how concerned she was about what was going on with Jake. She had been much more optimistic at his one-year check up which took place 3 months ago. At that time it seemed Jake was about to take off and start walking.
Now at 15 months, she was concerned because Jake wasn't walking yet. He just stopped trying. As the conversation continued, Karen reported that Jake is generally quiet and not very active. He's slow in initiating movements and his movements are clumsy when he does execute them. He's having some trouble eating finger foods. He is not very assertive in seeking attention or indicating his needs. He doesn't cry much and he doesn't seem unhappy. In fact, she wasn't quite sure about whether or not to have his ears checked because she couldn't tell how uncomfortable he was. She only observed that he woke up in the middle of the night more than usual. Karen commented that he seems like an easy child in so many ways.
Because Jake is Karen and Jim's first child, she wasn't sure what to make of her observations. She felt a tremendous amount of relief talking with Dr. Blake about her concerns. As it turned out, Jake did have an ear infection for which Dr. Blake prescribed the typical course of action. In addition, Karen talked with the office nurse about making a referral to the local Birth to 3 program.

Melanie has been home from the special care nursery where she spent the first 4 months of her life for nearly a month now. Jason, Melanie's dad, called the Birth to 3 program to say they were ready for a visit. Jason and Ellie had met the Birth to 3 team during a hospital discharge planning meeting. They were interested in following up with the Birth to 3 program, but wanted to spend some time at home as a family before opening their doors to service providers. Melanie was born prematurely at 28 weeks gestation and was very sick for the first 3 months of her life. She was diagnosed with asthma and required an NG tube for feeding. Now at 5 months, she has a G-tube for feeding and continues to have motor delays and significant tactile sensitivities. Melanie is very fussy and cries a lot. The Bayley was completed when Melanie was 3 months old by hospital therapists who worked closely with the special care nursery.
Ellie gave up an active professional position to be home with Melanie. She is committed to being home with Melanie and is finding many ways to enjoy being with Melanie. At the same time she is having trouble adjusting to the reduced pace of her life such as spending at least 30 minutes just to complete a feeding. In fact, one of Ellie and Jason's goals for Melanie is for her to become an oral feeding. They know this will take a lot of concentrated effort. Ellie has two, stepchildren in elementary school who are very helpful when they are at home.

Eduardo was referred to his county's Birth to 3 program when he was 10 months old. Anita, his mother was very concerned about his development, especially his motor and communication development. He wasn't rolling and he would try to sit up and after 15 seconds would fall over without catching himself. Anita remembered that her two older children acted differently at this age. Because Anita is a single mother who works about 30 hours a week, all three of her children are in child care with a neighbor. Just a couple of days ago Anita's neighbor asked her if she had noticed how much Eduardo falls over when he tries to sit up. After hearing her concerns echoed by her child care provided, Anita decided she wanted to get help. Her child care provider helped her find the number for the local Birth to 3 program. One of Anita's concerns in contacting the program was that she speaks English as a second language. She was worried about talking about such a personal topic with someone who didn't speak Spanish or come from a Latino background. In addition, a strong family priority is to preserve the Spanish language while also learning to speak English. She was very worried about what this means for Eduardo if he has developmental problems. What's she afraid to say to anyone is that she thinks he has some unusual physical characteristics that don't resemble those of other people in the family.
