Youth Transition to Adult Health Care

A FREE educational event for teens ages 14-21

This interactive workshop offers youth and their families an opportunity to explore how youth with disabilities or special health care needs can prepare for greater involvement in managing their own health care as they approach adulthood. Two separate sessions will be offered, one for parents and one for youth.

2007 Medical Home Summit: Small Steps, Big Changes

On November 15, 2007, over 140 participants gathered in Waukesha, Wisconsin for the 2007 Medical Home Summit. Highlighted by Dr. Carl Cooley’s keynote address, it was a day full of quality presentations and networking around medical home concepts replete with valuable resources. We thank the many presenters for their time and energy and the participants for their interest. We hope the summit showed some of the small steps that will eventually lead to big changes around medical home spread.
