Description

....Exploring ways to support youth with special health
care needs and disabilities as they transition from
pediatric health care and high school to adult health care, higher education, working and living within their communities.

Adolescent Transition and HRTW
Nationally, successful transition is being recognized as critical to improving the health, employment and social connections of young adults with special health care needs and disabilities. The Division of Services for Children with Special Health Care Needs (CSHCN), of the Maternal and Child Health Bureau (MCHB) has a ten year action plan with six outcomes to create a comprehensive system of care. Each state has a CSHCN Program and is addressing the same six outcomes. The sixth outcome states, “All youth with special health care needs will receive the services necessary to make appropriate transitions to all aspects of adult life, including adult health care, work and independence.”

In Wisconsin, our CSHCN Program is addressing the ten year plan in several ways. First, we have five Regional CSHCN Centers to address all six outcomes and to provide information and support to families and providers. Second, we are one of five states who has a Healthy and Ready to Work (HRTW) project to develop a model for improving adolescent transition within the framework of the CSHCN Regional Centers. During this four year period HRTW works with each Regional Center in pilot communities to improve the transition process by providing timely information, training, and support as youth approach adulthood.

Purpose of Wisconsin HRTW

  • Expand the capacity of the Children with Special Health Care Needs Regional Centers to provide information and support to youth and families in transition
  • Increase the number of youth who have a medical home and a transition plan from pediatric to adult health care
  • Increase the capacity of communities to support youth with special health care needs
  • Build family, youth and provider skills related to necessary components of transition
  • Connect statewide organization around the area transition

HRTW Activities
Community Development

  • CSHCN staff and HRTW community facilitators work
    on issues of health, independence, advocacy, and
    person-centered planning through community forums
    and training in HRTW pilot communities.

Youth Leadership

  • HRTW Youth Advisory Groups
  • Youth Speakers Bureau
  • Youth Conference Planning Committee

Person-Centered Planning

  • Youth and their families in pilot communities have
    the opportunity to participate in a person-centered
    planning process.

Information and Resources

Statewide Transition Consortium

  • Professionals, consumers, and parents share resources, ideas, and expertise as well as collaborate to enhance existing efforts and to initiate new activities.
