....Exploring
ways to support youth with special health
care needs and disabilities as they transition from
pediatric health care and high school to adult health care, higher
education, working and living within their communities.
Adolescent
Transition and HRTW
Nationally, successful transition is being recognized as critical
to improving the health, employment and social connections of
young adults with special health care needs and disabilities.
The Division of Services for Children with Special Health Care
Needs (CSHCN), of the Maternal and Child Health Bureau (MCHB)
has a ten year action plan with six outcomes to create a comprehensive
system of care. Each state has a CSHCN Program and is addressing
the same six outcomes. The sixth outcome states, All youth
with special health care needs will receive the services necessary
to make appropriate transitions to all aspects of adult life,
including adult health care, work and independence.
In Wisconsin, our CSHCN Program is addressing the ten year plan
in several ways. First, we have five Regional CSHCN Centers to
address all six outcomes and to provide information and support
to families and providers. Second, we are one of five states who
has a Healthy and Ready to Work (HRTW) project to develop a model
for improving adolescent transition within the framework of the
CSHCN Regional Centers. During this four year period HRTW works
with each Regional Center in pilot communities to improve the
transition process by providing timely information, training,
and support as youth approach adulthood.
