You and your family can enroll in our Research Registry in 2 ways:
- On-line Request
Complete the request form with your name, address, and contact information. The RPC will mail a consent form for you to complete and return.
- Print Consent Form
Print a copy of our consent form and mail the completed form to the RPC.
