About this Training: The Wisconsin Transition Improvement Grant (WITIG) supports youth moving into adulthood by providing technical assistance, training, and resources to educators, students, and families to improve post-school outcomes like employment, post-secondary education, and independent living. It does this through online tools such as the WiTransition App to help with self-advocacy and planning, guides like “Opening Doors” to understand adult services, and professional development for stakeholders to implement better transition practices. The grant also fosters collaboration with external agencies and encourages parents and students to be actively involved in transition planning to build skills for independence.

About The Series:  Transition Talks Tuesdays is a collaborative effort to provide students, families, educators, and community members with important information they need to know before a student turns 18 years old. We will cover a number of major areas that need to be considered. This fall we are inviting in some partners you might not be aware of and asking them “how do you support adolescents moving into adulthood and their families?”

Other Events in the 2025 Series:

Who should attend? Family members, self-advocates, educators, social workers, therapists, providers, community members – anyone who wants to understand some of pieces involved in becoming an adult if you have a disability or special health care need.

Presented by: The Wisconsin Youth Health Transition Initiative at the Waisman Center.
